What is home health care?
Home health brings skilled nursing, therapy, and care education into the home. Services are based on provider orders, assessed needs, and an individual plan of care. It is different from ongoing companionship or housekeeping.
Do you accept Medicare?
Yes. Vanguard accepts Medicare. Coverage depends on Medicare requirements, the ordered services, and individual eligibility. Call to discuss your referral and the information needed for a review.
Can I use insurance or private pay?
We accept most major insurance plans and private pay. Participation, benefits, authorization requirements, and patient costs vary by plan and service. We review these details before services begin.
Can I call before leaving the hospital?
Yes. Patients, families, and discharge planners may contact us while a discharge is being planned. Have the expected discharge date, destination address, and any home health orders available if possible.
What should I have ready when I call?
Helpful details include the city or ZIP code where care is needed, the referring provider’s name, discharge instructions, insurance information, and the type of skilled help recommended. You can still call with questions if you do not have everything yet.
