Help understanding your options

Returning home from a hospital, rehabilitation center, or skilled nursing facility often brings several changes at once. You may be arranging equipment, reviewing prescriptions, and planning follow-up appointments. Contacting us while discharge is being planned gives the team a chance to review the referral, location, coverage, and requested services.

We serve Los Angeles, Orange, Kern, and Ventura Counties. Tell us where care is needed, which provider is involved, and whether there is an anticipated discharge date. You can ask questions before you have all the paperwork.

Read about coverage and eligibility

Getting started, one conversation at a time.

We help you understand what comes next, before services begin.

1

Tell us what has changed.

Share the city where care is needed, any recent discharge plans, and the skilled services your provider has recommended.

2

Review care and coverage.

We discuss the referral, service availability, and Medicare, insurance, or private-pay arrangements. Specific benefits and costs need individual review.

3

Plan the next step.

Our team explains what information is still needed and how provider-ordered care can be coordinated when appropriate.
