Website Privacy Policy
Information you choose to share
The general inquiry form asks for your name, phone number, city, optional email address, and permission to contact you. Please do not use it to send medical records, diagnoses, insurance member numbers, or other sensitive clinical information.
How inquiries are used
Information submitted through the form is used to respond to your request, discuss service availability, and explain next steps. A successful submission is shown only after the website receives confirmation from its inquiry-delivery service. If delivery is unavailable, the website asks you to call or email instead.
Website service providers
Website hosting and inquiry-delivery providers may process information needed to operate the site and route a request to Vanguard. The static website does not create a form session cookie. The inquiry-delivery provider may apply its own security and retention practices. Contact the office if you prefer to make an inquiry by phone.
Website measurement
This website uses Google Tag Manager to manage measurement tags. Tags configured through it may collect website activity and device information or use cookies. The inquiry form does not add your name, phone number, email address, or city to its analytics event.
Website information and clinical records
This policy describes general website inquiries. It is not a Notice of Privacy Practices for patient medical records. Ask the care team for information about clinical records and the privacy practices that apply to your care.
Questions about your inquiry
Contact info@vanguardhhc.com or call (818) 617-9560 if you have questions about information you submitted, need to correct it, or wish to stop follow-up about a general inquiry.
Updates
This website policy may be updated when website features or information-handling practices change. Last updated September 16, 2026.
